This year, we are strongly encouraging prospective AmeriCorps sites to submit applications via email. Some documents, however, will still need to be submitted via mail.  If you are not comfortable with the electronic version, you can still use the downloadable "Word" version below. 

If you are applying via email, the Microsoft Word document is set up as a locked “form” so that you can easily click on the gray box and insert your answer. Download our document and save it to your computer. You can complete the application on your computer at your leisure. When you are ready to submit it, attach the application, along with any other materials you have in an email to jkirker@hhck.org. If you need technical assistance, contact Josie at (502) 223-1834.  The signature page must be forwarded by mail.

 If you are not comfortable completing the application process this way, a hard paper copy of the application may be mailed to: HHCK – GTD4KY, Site Application Review Team, 101 Burch Court, Frankfort, KY  40601

SUBMISSION DEADLINES: 

In order to help with our planning, we ask that you send an email to Josie (jkirker@hhck.org) by Friday, April 25 letting us know if you intend to apply, how many positions and what type of positions. By no means does this mean you have to apply, you can withdraw at any time. 

In order to select sites and begin recruiting members, this application is due to HHCK no later than Friday, May 9 with all attachments included.

Your agency will be notified of acceptance or rejection by June 6 at the latest.

NOTE:  The person who will act as the Site Supervisor for the AmeriCorps member MUST attend the Site Supervisor Training on June 19, 2008.  Please plan to attend the training now, as there is little time between site selection and the actual training.
